	As Adopter, I agree to the following 1. To allow a representative of Dogs Deserve Better to visit my premises to insure the terms of this agreement have been kept 2. The Animal will not be kept outdoors; 3. The Animal will not be kept on a chain; 4. The Animal will be provided with adequate fresh food and water, clean, dry shelter when outside, and daily exercise; 5. To provide a safe collar with rabies and I.D. tags to be worn at all times. 6. To obey all applicable laws governing control and custody of animals; 7. To provide all medical care and treatment needed by the Animal including but not limited to: yearly medical checkups and vaccines, and preventative heartworm medication; 8. To adopt the Animal only as a personal pet/companion and not as a gift, working animal, or guard dog; 9. The Animal is not be sold or given away for any reason. If it becomes necessary to find the Animal another home, Dogs Deserve Better will be contacted. Pet ownership is a serious commitment that the entire household needs to consider and agree to before the animal is adopted. We want to ensure that each adoptive household is aware of and willing and able to accept the physical and financial responsibilities of pet ownership. Not everyone that desires a pet is ready to properly care for one. I understand all of the questions/statements and have completely read the adoption application and understand that this Dogs Deserve Better dog MUST be indoors and this dog will not be chained/penned outside. I further understand that if for any reason I want to give up this dog, I will call Dogs Deserve Better, and undergo a 14 day waiting period in which representatives of DDB work with me to resolve any issues with the dog. I also understand that a representative from the organization may visit the dog to ensure that it is well taken care of; if we find that the dog has been neglected or abused in any way or for any reason, we reserve the right to remove the dog from the premises. Your signature below authorizes us to do so as necessary, and automatically transfers ownership rights of said dog back to Dogs Deserve Better at that time. Dogs Deserve Better assumes no responsibility nor shall they be held liable for any damages caused by the adopted Animal to property, person or other pets. If any medical problems should occur after adoption, Dogs Deserve Better is not liable for any medical bills. As Adopter, I agree to accept full responsibility for the care of this animal releasing the previous owner as well as Dogs Deserve Better and its representatives of any liability from this date forward. I agree to abide by the adoption conditions and realize that I am committed to this animal for his/her life. I have read and fully understand that Dogs Deserve Better may repossess this animal at any time if the adoption conditions are violated or the animal is mistreated.
